Eligibility Criteria
The care recipient must be at least 16 years old.
They must have MassHealth Standard or CommonHealth insurance. Please contact us if you have another insurance provider to confirm eligibility.
Loved one should require daily assistance with activities such as eating, bathing, or dressing.
A primary care physician must provide a written order for AFC services.
Caregiver Requirements
Must be at least 18 years old.
Can be a relative (e.g., adult child) but cannot be the care recipient’s legal guardian or spouse.
Must reside with the care recipient and be responsible for their daily care.
